How can I advise an enrollee to choose me as their PCP if they have selected or been assigned to someone else? Please have the enrollee call Customer service at 1-800-876-9758 and ask to change their PCP selection. You can also have them complete a PCP change form and fax the request to 1-901-346-3376 . Remember, when using the PCP change form, the enrollee must sign the form in order for us to make the change.
