Disease Management

UAHC Health Plan has the following Disease Management (DM) Programs available:

  • Asthma
  • Congestive Heart Failure
  • Diabetes
  • Maternity Care (High-Risk Pregnancy)

We’ll provide your patients with:

  • Access to a dedicated, experienced registered nurse to call for guidance and support; Nurses provide answers to questions about symptoms and direct the patient to the appropriate level of care.
  • Educational material regarding their condition to ensure that each patient understands his/her condition and how to best manage it.
  • Self-care information and reminders of important tests and exams to assist your patient inunderstanding and managing the treatment and medication regime and activities that you prescribe.
  • Informational newsletters to keep your patient informed of the latest information on his/her disease or condition.

The overall goals of the DM programs are to enhance communication between practitioners and patients, facilitate feedback necessary for behavior modification (which may prevent or delay disease progression), and measure the effectiveness of interventions. UAHC’s goal is to provide education and resources that will assist these members in managing their health and improving the quality of their lives.

Each program allows the physician to design a personalized action plan for the patient. Once a member is enrolled in a disease management program, his or her physician and other health care providers also receive support and education from UAHC. Disease Managers interact with physicians and provide ongoing clinical updates and dialog regarding the member’s disease management plan. Physicians also receive a welcome kit with clinical practice guidelines, samples of educational tools given to their patients, and access to a toll-free number to reach UAHC staff. The physicians receive a quarterly newsletter and are invited to evaluate outcomes and support communication within the program.

Members do not have to do anything to join. Members are automatically enrolled when they are identified through authorizations or claims.
